BPPM 13:330 establishes University policy on Cellular/Wireless Service.

There are four forms depending on the option the department selects for that employee/phone.

Agreement Forms

1 - Cell Phone & Cell Phone Service Paid By University

2 - Employee Owned With No Reimbursement for Business Use

3 - Employee Owned With Reimbursement of Business Use

4 - University Owned PDA/Employee Provided Service
