BPPM 13:330 establishes University policy on Cellular/Wireless Service.

There are four forms depending on the option the department selects for that employee/phone.

Agreement Forms

  1. Cell Phone & Cell Phone Service Paid By University
  2. Employee Owned With No Reimbursement for Business Use
  3. Employee Owned With Reimbursement of Business Use
  4. University Owned PDA/Employee Provided Service
